Accident and damage report
You can find the accident report form in the MyFleet application, under Fleet Operations/Accident & Damages, by entering the registration number in Latin characters. To log in click here.
We suggest that you always have a declaration of accident form available in your car. You never know when it might prove useful! You can find it here.
If your lease car has been damaged, you must report this to Ayvens as soon as possible. You can easily report a damage by calling the Driver’s Help Line +30 2106100050 and follow the instructions.
Accident Care will proceed to all the necessary actions on your behalf.
If for some important reason you choose not to accept the Accident Care services, then you must either fill out the declaration of accident form that can be found in your car - you can download it here, or exchange contact details with the other involved driver, if you do not have any form available. Please use your mobile phone and take pictures of both vehicles. Make sure to write down the involved car’s license plate, as well as the collision point of both the cars and the area. Please remember that every information about the accident circumstances is valuable to defend your position.
